 Job Description/Requirements: The Signature Home Health RN Case Manager will cover a geographic territory in the Tacoma/Federal Way, WA, providing nursing services to a variety of patients with variety of medical conditions in accordance to the physician plan of treatment. You will be working with a comprehensive team of medical professionals who meet once a week to discuss plan of care on each patient. The Home Health RN will prepare and complete clinical and progress notes the day of services; coordinates medical services; informs the physician and other medical personnel of changes in the patient’s condition and needs.  The RN will provide education to patient and family providing complete nursing services that will enhance the life of the patient.
