Full-time, part-time and PRN positions are available. (EOE)


The Occupational Therapist provides rehabilitative care for patients needing assistance and training in activities of daily living. The Occupational Therapist assists the physician in evaluating level of function, helps develop the plan of care (revising it as necessary), prepares clinical progress notes, advises and consults with the family/caregivers and works as a part of the interdisciplinary team.

Knowledgeably and competently delivers occupational therapy to patients

Evaluates patient needs through ongoing assessment and revises care plans based on changes in patient condition

Documents and charts appropriately and within specific time frames

Follows physician orders and occupational therapy treatment plans for patients

Oversees and evaluates care given by COTAs (and HHAs if state law allows)

Identifies equipment needs of patient, requesting physician orders for equipment, working with equipment companies in ordering equipment for patient, and educating patient and caregiver on new equipment

Regularly re-evaluates patients to identify progress towards goal achievement; reviews OASIS answers, coding and sequencing of diagnoses to ensure accuracy; obtains orders and updates Plans of Care as needed throughout the episode of care.

Participates effectively as part of an interdisciplinary team to assure that patient services are delivered in a timely manner

Requirements

EDUCATION and EXPERIENCE

Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association

If does not meet above education criteria, must be eligible for the National Registration Examination of the American Occupational Therapy Association or must have 2 years appropriate experience as an Occupational Therapist along with a satisfactory grade on a proficiency exam conducted, approved or sponsored by US Public Health Service

Minimum of 1 year experience as an Occupational Therapist, or, minimum of 2 years experience with proficiency exam

Home Health experience preferred

CPR certification

SPECIFIC REQUIREMENTS

Valid drivers license and auto liability insurance

Reliable transportation to travel to patient homes per patient schedule and agency needs

Knowledgeable of occupational therapy practices and procedures based on state practice acts

Ability to make independent clinical decisions
